Terms and Conditions

I was hoping someone could help me with this problem I'm having?

Basically, I want to add a Terms and Conditions field on the signup forms. I have created the field and it does show up on my forms, but it doens't have a check box (and I've selected the check boxes option when creating the field). I've tried everything to make the check box show up but it doesn't. At first I thought maybe it was a template problem (I'm using a nasthon templte) but I tried various templates and all of them have the same issue.

Anyone else run into this problem? If anyone has any advice please tell me what I am doing wrong?
